Judgement is finite. A person can hold only so many relationships in which they genuinely know what is happening and can be held to it. Add people, add layers, add systems that decide faster than anyone can review, and oversight thins without announcing itself. The forms still get signed. The limit is relational, not technical, and no tooling repeals it. Past that point an organisation is not supervising its decisions. It is receiving reports about them.
